Both Federal and State law require Illinois State University to periodically report information about the University’s workforce and equal opportunity efforts.

To assist in reporting, the Office of Equal Opportunity, Ethics, and Access requests all faculty and staff complete the Voluntary Self-Identification Survey by logging in to and navigating to the survey in the Announcements Section. We ask that all employees, even those who completed the survey previously, complete this new survey as some of the information we are required to report has changed.

Completion of this survey is strictly voluntary. Disclosure or refusal to provide information will not result in adverse treatment. Responses will be kept confidential and all information will be used only in a manner that is consistent with the University’s Federal and State reporting requirements. Responses will not be used to determine eligibility for other University programs and/or processes which may require self-identification. Please review the Frequently Asked Questions document for more information about the survey.

If you are unable to complete the survey electronically, need to request an accommodation to complete the survey, or if you have questions about the survey, contact the Office of Equal Opportunity, Ethics, and Access at (309) 438-3383.
